Đặt vấn đề: Thiếu máu tán huyết miễn dịch (TMTHMD) là bệnh tự miễn nguy hiểm, trong đó một tỷ lệ đáng kể bệnh nhân kháng hoặc tái phát sau điều trị corticosteroid. Rituximab hiện được sử dụng như liệu pháp bước 2, tuy nhiên dữ liệu trong nước còn hạn chế. Nghiên cứu này được thực hiện nhằm đánh giá ban đầu hiệu quả điều trị bước 2 bằng Rituximab ở bệnh nhân TMTHMD nguyên phát và thứ phát tại Bệnh viện Truyền máu Huyết học.
Đối tượng và phương pháp: Nghiên cứu mô tả loạt ca đánh giá đáp ứng điều trị trên 10 bệnh nhân TMTHMD nguyên phát và thứ phát, không đáp ứng hoặc tái phát sau điều trị corticosteroid, được điều trị Rituximab bước 2 từ tháng 10/2019 đến tháng 10/2025. Các biến số được thu thập gồm đặc điểm lâm sàng, cận lâm sàng trước điều trị và đáp ứng điều trị tại các thời điểm 1, 2, 3, 4, 8 và 12 tuần sau truyền Rituximab.
Kết quả: Độ tuổi trung vị của nhóm nghiên cứu là 44,0 tuổi (tứ phân vị: 33,0 – 69,5), nữ giới chiếm tỷ lệ áp đảo 80,0%. Toàn bộ bệnh nhân (100,0%) mắc TMTHMD thể nóng, với tỷ lệ bệnh nguyên phát và thứ phát tương đương nhau (50,0%). Trước điều trị, tình trạng thiếu máu nặng ghi nhận với Hemoglobin trung vị là 6,80 g/dL. Sau điều trị bằng Rituximab, đáp ứng lâm sàng cải thiện rõ rệt theo thời gian: tại tuần thứ 8, tỷ lệ đáp ứng một phần (PR) đạt 60,0%, tăng đáng kể so với mức 50,0% ở tuần thứ 4 (p = 0,028). Đến tuần thứ 12, tỷ lệ đáp ứng chung (ORR) đạt mức 80,0%, trong đó có 30,0% bệnh nhân đạt đáp ứng hoàn toàn (CR). Nồng độ Hemoglobin cải thiện có ý nghĩa thống kê tại tuần 8 (p = 0,0144) và tuần 12 (p = 0,0309) so với trước điều trị.
Kết luận: Rituximab cho thấy hiệu quả ban đầu trong điều trị bước 2 TMTHMD nguyên phát và thứ phát do bệnh tự miễn, giúp cải thiện Hemoglobin và tăng tỷ lệ đáp ứng trong thời gian theo dõi 12 tuần.
Abstract
Background: Autoimmune hemolytic anemia (AIHA) is a serious autoimmune disorder in which a substantial proportion of patients are refractory to or relapse after corticosteroid therapy. Rituximab is currently used as second-line treatment; however, domestic data remain limited. This study aimed to conduct an initial evaluation of the effectiveness of second-line Rituximab therapy in patients with primary and secondary AIHA at the Blood Transfusion and Hematology Hospital.
Methods: This descriptive case series study evaluated treatment responses in 10 patients with primary and secondary AIHA who were refractory to or relapsed after corticosteroid therapy and subsequently received Rituximab as second-line treatment between October 2019 and October 2025. Clinical and laboratory characteristics before treatment and treatment responses at 1, 2, 3, 4, 8, and 12 weeks after Rituximab administration were collected.
Results: The median age of the study population was 44.0 years (interquartile range: 33.0–69.5), with a predominance of female patients (80.0%). All patients (100.0%) were diagnosed with warm autoimmune hemolytic anemia, with an equal distribution between primary and secondary disease (50.0% each). At baseline, severe anemia was observed, with a median hemoglobin level of 6.80 g/dL. Following Rituximab treatment, clinical responses improved progressively over time. At week 8, the partial response (PR) rate reached 60.0%, representing a significant increase compared with 50.0% at week 4 (p = 0.028). By week 12, the overall response rate (ORR) was 80.0%, including 30.0% of patients achieving a complete response (CR). Hemoglobin levels showed statistically significant improvement at weeks 8 (p = 0.0144) and 12 (p = 0.0309) compared with baseline.
Conclusion: Rituximab demonstrated initial effectiveness as second-line therapy for primary and secondary AIHA, leading to significant improvement in hemoglobin levels and increased response rates during the 12-week follow-up period.
